I am unable to accept any direct payment from New Patients if you have Medicare. Do not book a New Appointment if you have Medicare. If you would like to book a capacity evaluation then another person (with capacity) who knows you must call and speak with me on the phone about payment and must accompany you to the appointment. I do not accept insurance or work for insurance companies but I can provide you with an itemized Superreceipt that your insurance company (unless you have Medicare) accepts for out-of-network benefits.Please click 'Book Now' for either a New or Follow-Up Appointment. I can only see residents of TN, KY and AL. All Telehealth New Visits must have the New Patient Packet emailed to be before the visit (email at top of New Patient Packet). You can either download, print and fill out our New Patient Form to bring with you to your first appointment or arrive early
